Step 1
Print out the Offshore Visa Debit Card - Application Form provided below, using Adobe Acrobat.
Complete the application form.

Step 2
Once you have completed the form ensure that you have signed this (including the box for laser etching)

Step 3
Attach a black and white or colour photograph (size 30 mm x 35mm) for each applicant.

Step 4
The completed Offshore Visa Debit Card application duly signed together with the photographs, should be forwarded to either Standard Bank Jersey Limited or Standard Bank Isle of Man Limited, depending on where your account is held.
