To help us do so, we have prepared a sample personal banking account application form which gives an indication of how we would like you and your clients to complete application forms before submitting them to the bank. At the end of the form there is a flowchart covering the issues of ensuring that evidence of identity and proof of address for the application meet the standards required by the bank.
